Medication Therapy Management or MTM is the evaluation of medication therapy to improve continuity of care and health care outcomes. MTM is an important service for Seniors because it prevents the 1.5 million adverse events that occur each year that cause $177 billion in injury and death.

There are many age-related changes that can have a significant effect on how drugs work in the senior patient such as:
1. Multiple Chronic Diseases- 80 percent of seniors have at least one chronic disease, and many of them have multiple diseases.
2. Multiple medications - (Polypharmacy): On average, seniors take as many as three times the medications as younger patients, increasing the potential for adverse drug events.
3. Increased Side Effects: Seniors experience side
effects at a rate of two to three times greater than that of the younger adult. This increased risk is due to changes in kidneys and the liver, which will ultimately affect how medications work in the body.
